Trader Jimmy isn't a fan of the tutu trend!

Trader Jimmy is almost three years old. We got him a little over a year ago. Not long after, a wound started appearing on his head. Thinking it was a hot spot, I took him to the vet. They prescribed antibiotics. It healed, then happened again. Took him to a different vet. They shaved the area, prescribed antibiotics, and put cream on it.
Neither vet gave a specific diagnosis for what's going on or causing this to happen. I ended up taking matters into my own hands and purchased wound spray. Every time the wound opens, I spray until it heals.
I've personally never seen Trader Jimmy scratching at his head. So he must be doing it when I'm not around. He doesn't whine or cry about it but the spot is tender to the touch. The area keeps getting bigger every time. He's gonna have his brain dug out soon!
Do any of you have an idea of what could be causing this? Why does my cat feel it's necessary to walk around sporting a gash? I know it's not cuz I made him wear a tutu!
